GENERAL ELECTRIC CF34 10E5A1—ATA 7830 THRUST REVERSER THRUST REVERSER INTERMITTENT. DR 611649057
Thrust reverser failed to deploy in landing phase. Safely considered one-off condition. Nil faults confirmed after troubleshooting. Maintenance records reviewed with no previous history. CMC BITE carried out, no maintenance messages found recorded. Thrust Reverser function test carried out IAW AMM 78-30-01-XX, found satisfactory. Thrust Reversers #1 and #2 operated normally through open and stow commands. There have been no repetitive occurrences. Event monitoring will continue.

GARRETT TPE331 1—ATA 7210 TURBINE ENGINE REDUCTION GEAR TURBINE ENGINE INFLIGHT SHUTDOWN. DR 611649058
Uncommanded IFSD. Indications didn’t lead to an obvious cause to the failure. Root cause under investigation at this point, as it is internal to the gearbox and will be subject to strip and inspection by an approved engine OEM repair shop. Preliminary engineering observation indicate that the engine has uncoupled from the propeller shaft within the Gearbox.

ATA 7230 TURBINE ENGINE COMPRESSOR SECTION—ROTOR UNSERVICEABLE. DR 611649070
During inspection of a DISK HPC STG. 10, seal wire groove, showed damages beyond repair. DISK HPC STG. 10 rendered unserviceable and will be scrapped.

PRATT & WHITNEY PT6A 65AG—ATA 7230 TURBINE ENGINE COMPRESSOR SECTION COMPRESSOR STALLED. DR 611649083
Engine developed an issue on take-off power roll. Pilot aborted take-off. No damage or accident occurred as a result to Airframe. Possible first stage compressor stall. OEM field representative have begun initial investigation and engine will be sent to approved repair station.

ROLLS-ROYCE TAY650 65015—ATA 7200 ENGINE (TURBINE/TURBOPROP) TURBINE ENGINE VIBRATES. DR 611649096
The #1 engine Hi VIB caution appeared during take-off roll approximately 3-5 seconds after advancing thrust levers to EPR 1.3. QAR downloaded. Vibration survey carried out nil high vibration evident. MCD checked clean. Investigation is continuing.

ROLLS-ROYCE TAY650 65015—ATA 7200 ENGINE (TURBINE/TURBOPROP) TURBINE ENGINE VIBRATES. DR 611649115
In climb phase, LH #1 Engine vibration ‘N2 HI VIB’ alert presented on the Multifunction Display Unit (MFDU). Crew selected ‘ALTN VIB’ with no change to the vibration level. Thrust was reduced to bring Vibes into acceptable limit and a safe air turn back was initiated.
During troubleshooting, IDG and FFR were replaced. As vibes did not improve, HP fuel pump replacement planned. Technical Services are also consulting with Rolls-Royce and troubleshooting is continuing.

PRATT & WHITNEY PW127—ATA 7200 ENGINE (TURBINE/TURBOPROP) O RING DAMAGED. DR 611649143
On starting #1 engine, oily smell and haze was noticed in the flight deck and cabin. Bleed and Packs selected OFF and fumes began to dissipate. Investigation concluded cause of the defect was oil leaking through the NL probe O-Ring. NL Sensor removed, O-Rings replaced and sensor Re-installed IAW MM PWC XX. Engine Ground Runs Carried out satisfactory with nil abnormal smells noted. There have been no further reports of oil smell. This is the first occurrence of this type on operator’s experience. NL probe O-Rings are replaced at every Engine Shop Visit. Nil reliability issues are current.

GENERAL ELECTRIC GENX1B64 P2—ATA 7311 ENGINE FUEL/OIL COOLER HEAT EXCHANGER LEAKING. DR 611649167
While accomplishing SB 79-0031 on #1 engine main fuel/oil heat exchanger, fuel was found to be leaking from the main fuel/oil heat exchanger transfer tube. Quantity of fuel was insufficient to become a source of fire hazard.

ATA 7200 ENGINE (TURBINE/TURBOPROP)—TURBINE ENGINE BIRD STRIKE. DR 611649183
During walk around, suspected bird strike on left nacelle/propeller noted. Suspect bird strikes were crushed bugs. Inspected and cleaned with nil defects evident.

PRATT & WHITNEY PT6A 67AG—ATA 7321 FUEL CONTROL/TURBINE ENGINES FCU SUSPECT FAULTY. DR 611649239
Engine power reduced to idle during flight. FCU was replaced. Removed unit sent to approved repair facility.
