BELL 206 B–ATA 5330 FUSELAGE MAIN, PLATES/SKIN–SKIN–CRACKED. DR 611749984
During inspection, inner skin found cracked @ roof panel at the forward LHS transmission mount. Possible cause could be aircraft age/fatigue. Repair will include installation of a doubler in accordance with a part 21M Engineering order.

BELL 206 B–ATA 7120 ENGINE MOUNT SECTION–BULKHEAD–CRACKED. DR 611749985
The LHS aft engine mount support/C-frame is cracked at two rivet holes. Age/fatigue–aircraft has not been upgraded. Replace the C-frame in accordance with the Bell Helicopter SRM.

BELL 206 B–ATA 6210 MAIN ROTOR BLADES–MAIN ROTOR BLADE–De-bonded. DR 611749986
Outboard trim tab has de-bonded from the blades lower skin. The inboard upper doubler plate is de-bonding from the upper blade skin. Possible cause could be erosion of the bond line, moisture ingress. Approved repair planned.

BELL 206 B–ATA 6500 TAIL ROTOR DRIVE SYSTEM–CONTROL TUBE–SCRATCHED. DR 611749987
Scratched control tube due incorrect fastener installation. Replacement of control tube planned. Bell AD/BELL 206/40 embodiment to follow.

ROBINSON R22 BETA–ATA 6220 MAIN ROTOR HEAD–BEARING–WORN. DR 611750014
At the 500 hour spindle inspection per AD/R22/30 Amdt 5 and SB 60A the spindle bearing were found very rough. 1 race was found brinelled. New spindle bearing p/n 7116 kit installed.

During transit of the aircraft, main transmission chip light illuminated. Chips found on chip detector during inspection. Main transmission overhauled.

BELL 206 B–ATA 6420 TAIL ROTOR HEAD–BEARING–CRACKED. DR 611750117
During T/R hub assy maintenance, the T/R blades were removed. Bearing found cracked during inspection. Investigation ongoing.

Corrosion observed on primary structure.

ROBINSON R66–ATA 6310 ENGINE/TRANSMISSION COUPLING–CLUTCH–UNSERVICEABLE. DR 611749965
During planned engine removal and replacement, sprag clutch was found to be unserviceable due to a lack of oil. Clutch was not due for replacement but had to be replaced. All oil was lost due to a plug coming loose. Robinson Helicopter Company advised that this was a known problem under investigation.

SIKORSKY S92–ATA 6400 TAIL ROTOR SYSTEM–Temperature Indicator Plug–Turned Black. DR 611749991
During TRPCS BA 10-15 FH borescope inspection, all four temperature dots on temperature indicator plug found to have changed colour to black. Replaced tail rotor pitch change shaft and bearings.

AGUSTA AW139–ATA 6720 TAIL ROTOR CONTROL SYSTEM–BONDING LEAD–Interfering. DR 611750036
During a visual inspection of the helicopter it was noticed that a bonding lead Pt No A537A01AA01-0380 was rubbing on the No#1 tail rotor driveshaft Pt No 3G6510A01132 with evidence of contact on the shaft and wear on the lead. The bonding lead is located within the FWD tail rotor drive shaft cover. The drive shaft was removed, cleaned, polished and inspected. Nil damage was visually found after this process. The driveshaft was also cleared of cracking by Eddy Current procedure as advised by OEM. The driveshaft was refitted and the bonding lead was also replaced with sufficient clearance confirmed during closure of the drive shaft cover.

Fuel smell/fumes in cabin and cockpit. Aux fuel tank vent hose found disconnected. Vent hose reconnected IAW A/C MM. Ground and flight checks C/O. Nil further defects.

AGUSTA AW139–ATA 2590 NON STANDARD EQUIPMENT SYSTEM–SQUIB–Not installed. DR 611750039
During phase 1 inspection (50 hours total time in service [TTIS]), the rescue hoist was inspected. When the hoist cover was removed, engineer identified that the hoist cartridge (squib) was not fitted and the lead was secured to the side. Once the issue was identified, all remaining aircraft with less than 50 hours TTIS were inspected to ensure squibs were fitted. No additional issues were identified.

AEROSPATIALE AS355 F1–ATA 6220 MAIN ROTOR HEAD–BOLT–CORRODED. DR 611750049
During carrying out ASB AS355-62.00.35 two main rotor head attachment bolts were found to be corroded. Bolts replaced.
