EUROCOPTER AS350 B2–ATA 2913 HYDRAULIC PUMP, (ELECTRIC/ENGINE), MAIN–PULLEY–DISLODGED. DR 611851707
During pre-flight checks, ‘HYD’ on CWP was noted. Initial inspection of the hydraulic pump showed pulley, drive belt attached to the pump to be off axis. Later, it was revealed that circlip securing Bearing race has dislodged allowing pulley to be easily removed by hand. Hydraulic pump & pulley assy were replaced with serviceable components. Drive belt was however serviceable.

ROBINSON R44–ATA 6210 MAIN ROTOR BLADES–MAIN ROTOR BLADE–CRACKED. DR 611851709
A crack was found when rotor blade was removed for painting. Crack had not yet broken through the paint and was very difficult to see.

BELL 412 EP–ATA 2497 ELECTRICAL POWER WIRING–sensor relay–FAULTY. DR 611851713
On departure, burning electrical smell was noted. #1 AP went offline, at the same time #2 artificial horizon off flag appeared. Engineering investigation found No 2 AV Voltage Sensor Relay to be faulty. Part was replaced.

AGUSTA AW139–ATA 2590 NON-STANDARD EQUIPMENT SYSTEM–Night Vision Goggles–LOOSE CONNECTION. DR 611851721
During approach, pilot’s NVG’s failed. NVG failure was rectified during the circuit by disconnecting then reconnecting the socket between the battery pack and NVG mount after confirming that the secondary batteries did not change the situation. NVGs were tested for functionality on the ground. It is believed that the increased vibration of 102%NR on approach may have been a contributing factor.

AGUSTA AW109 SP–ATA 5310 FUSELAGE MAIN, STRUCTURE–Fitting Plates Counter Torque–CRACKED. DR 611851765
During accomplishment of Service Bulletin 109SP-118 (ATA 53–Left and Right Counter torque crossbeam fitting plate, Assemblies P/N 109-0330-17 Inspection Of.), cracks were identified in the fitting plate assemblies on three aircraft, requiring the subsequent replacement of fittings.
