ROLLS ROYCE RB211 535E4 37–ATA 7250 TURBINE ENGINE TURBINE SECTION–HPT Shroud–WORN. DR 611851951
On pre-fight inspection, debris was found in the bottom of the No 2 engine exhaust case. Borescope inspection conducted on HPT-LPT-NGV found HPT shroud wear but no damage to any rotating critical parts. RR advised that this material loss has been seen in the past, but not of this magnitude. Moreover, as hot gases have ingested into the HP and IP discs, RR is unable to model the subsequent effect of temperature increases. Consequently, RR is unable to offer any further relief without quantifiable modelling, which in this case cannot be carried out. Decision taken to replace engine.

GENERAL ELECTRIC CT7 9B–ATA 7200 ENGINE (TURBINE/TURBOPROP)–TURBINE ENGINE–BIRD STRIKE. DR 611851963
On take-off, a group of four to five galahs flew out from the grass adjacent to the runaway directly in front of aircraft. On post-flight inspection, one bird was found to be ingested into RH engine. Moreover, one blade of RH propeller and the RH AOA vane were also impacted. Further inspection found damage to one stage 1 compressor blade and to one propeller de-icer boot. Engine will be removed and replaced as it requires cold section repair. AOA vane and propeller will be inspected, repaired or replaced.

CFM CFM56 7B 26–ATA 7830 THRUST REVERSER–THRUST REVERSER–FAILED TO DEPLOY. DR 611851965
Flight crew reported engine #2 reverser didn’t deploy after landing; engine #2 thrust reverser light illuminated. CB for engine #2 reverser sync lock power was found tripped. CB reset, reverser operational test was completed satisfactory in accordance with FIM Task 78-34/801. Review of historical records indicates affected circuit breaker was not disrupted during recent maintenance. There is no history and there have been no repetitive occurrences. Event monitoring will continue.

After take-off, airframe vibration felt. ADV displayed on RH Engine indicating 10.0 units of vibration. QRH procedure followed. At idle, vibration still 6.5 units. Number 2 Engine shut down. Upon arrival, significant turbine failure was witnessed. Exiting turbine pieces had caused secondary damage to flaps and body fairings.

During take-off, flight crew reported loud pop noise, slight yaw, followed by a momentary rubber smell in cabin and flight deck. All engine indications confirmed normal, ATB conducted with no issues. Bird strike was latter detected on engine #1. Relevant BSI completed, bird remains found on booster inlet section. No damage was witnessed. Bird remains cleaned, power assurance check carried out satisfactory. AMM Task 05-51-18-210-801 inspections logged, aircraft released to service.

Engine start panel noted by crew to be incorrectly labelled with ‘NORM’ in between the ignition switches, when it should be labelled ‘AUTO’. Internal investigation has been completed.
