MCCAULEY PROPELLERS D3A34C404–ATA 6122 PROPELLER GOVERNOR–GOVERNOR–INTERMITTENT. DR 611751165
Pilot, who delivered the aircraft for the 100-hourly inspections reported propeller not governing intermittently. Engine removed and sent for warranty repairs/claims.

DOWTY-ROTOL LIMITED R390/4-123-F/27–ATA 6123 PROPELLER FEATHERING/REVERSING–Autocoarsen switcher unit–FAULTY. DR 611751188
During post compressor wash auto coarse pitch selection task, LH propeller failed desired outcome i.e. auto coarsen switcher unit at fault. Same replaced and tested serviceable.

HAMILTON STANDARD 14SF-23/SFA13-OA–ATA 6111 PROPELLER BLADE SECTION–PROPELLER–OUT OF BALANCE. DR 611751206
Propellers suspected to be out of balance throughout all RPM settings except at 900 and 1200. Possibly could be due to a single blade change event, full spinner off propeller inspection carried out with nil faults. New balance weight solution applied to backing plate. Verification flight will be completed for confirmation of the defect resolution.

MCCAULEY PROPELLERS 3FF32C501–ATA 6122 PROPELLER GOVERNOR–GOVERNOR–CONTAM-METAL. DR 611751231
Upon applying power up to 25 Manifold Pressure, temp & pressures were stable. After low boost pumps selected, left engine RPM appeared to be bogged down. After brakes release and upon application of full power, RPM decreased rapidly. Throttles were closed and take-off was rejected. During ground run for troubleshooting, engine RPM decayed simultaneously with the propeller feathering and oil pressure dropping to zero. Filters showed significant amounts of metal. Engine replaced. Initial assessment indicates the likely source of metal contamination is the propeller governor. Governor has been sent to another facility for further strip & assessment.

MCCAULEY PROPELLERS 4HFR34C778-1–ATA 6112 PROPELLER DE-ICE BOOT SECTION–HOSE–MISSING. DR 611851251
During pre-flight inspection, small nicks were found on nose cowling and one of the propeller blades. Upon further investigation, it was found that the anti-ice system pipe was missing. Entire fleet spinners were removed and inspected. No other hoses or clamps were found missing. Security check of hose and clamps was carried out. Some of the clamps were found slightly loose, which were subsequently tightened and aircraft returned to service. A hose clamp inspection after every 50 hrs is scheduled until the hose clamp tightness has stabilised. McCauley propeller was notified of the occurrence.

Governor disassembled and inspected for metal contamination. Found idler gear bearing destroyed. Previous correspondence with McCauley regarding bearing installation on file.

DOWTY-ROTOL LIMITED R375/4-123-F/21–ATA 6114 PROPELLER HUB SECTION–Prop Gear Box–FAILED INSPECTION. DR 611851438
During scheduled visual inspection of RH PGB propeller shaft per SB72-531, corrosion around both dowel pins. Findings were sent to GE who confirmed our assessment that it was an unserviceable condition. Findings recorded in attached documents. PGB was removed & serviceable fitted. PGB has been shipped out for repair.
