SIKORSKY S92 A–ATA 2312 VHF COMMUNICATION SYSTEM–ANTENNA–BROKEN. DR 611852280
HF antenna found broken after landing. Wire repaired IAW S92 AMM Ch 23-14-XX. Follow-up test satisfactory.

SIKORSKY S92 A–ATA 3230 LANDING GEAR RETRACT/EXTENSION SYSTEM–SOLENOID VALVE–LEAKING. DR 611852283
Number 3 hydraulic pump fail indication, LG failed to retract. Defect traced to gas leakage through RH retraction actuator solenoid valve. Same replaced. Afterwards, during landing gear retraction test, it was discovered that LH MLG retraction actuator had failed, replaced with serviceable item. Follow-up tests satisfactory.

SIKORSKY S76 A–ATA 3110 INSTRUMENT PANEL–ANNUNCIATOR–CORRODED. DR 611852292
Reported annunciator light not seating. Initial test found several warnings inoperative. On removal of covers, evidence of water damage/corrosion found on printed circuit board as well as burnt tracks. Thorough cleaning of PCB completed. Repair of tracks, replacement of annunciator capsules done.

ROBINSON R66–ATA 2435 STARTER-GENERATOR–CLAMP–BROKEN. DR 611852301
On reinstallation of starter generator, crack (from the welded joint) noticed on clamp, new clamp fitted. Failure of clamp while fitted might have resulted in generator being detached from the engine. Which could result in catastrophic failure.

ROBINSON R66–ATA 8300 ACCESSORY GEARBOXES–FREEWHEEL–Notchy. DR 611852302
While removing clutch assy, it was observed that unit was very notchy when rotating the yoke. Moreover, pilot had reported vibration from somewhere. New Style FO18-1 was fitted.

AGUSTA A109 E–ATA 3197 INSTRUMENT WIRING–Indications–FLUCTUATES. DR 611852306
Flight crew reported #1 engine oil/hydraulic pressure indications not stable during flight. Relevant electrical connectors cleaned and reinstalled. Mechanical systems integrity checked OK. Aircraft released to service post maintenance check flight, satisfactory.

EUROCOPTER EC135 P2 PLUS–ATA 2590 NON-STANDARD EQUIPMENT SYSTEM–MOUNT–CRACKED. DR 611852328
Crack noted on quick disconnect mount during maintenance visit. Crack was not visible in-situ. Cause not able to be identified–nil abnormal loads/impacts noted–suspect age (10+yrs). Mount replaced (quarantined–to be scrapped). The remainder of the fleet inspected for same with nil anomalies noted.
