EUROCOPTER EC175 B – ATA 6320 MAIN ROTOR GEARBOX – MGB – CHIP INDICATION. DR 611953331
Intermittent MGB chip indications. HUMS download indicated multiple been 13 indications during hour flight. During inspection, “hair” like particle approximately 4mm in length were observed. Relative maintenance completed. OEM (Airbus Helicopters) contacted for further direction. Heavy hover checks carried out, material sent to OEM approved lab for further investigation.
SIKORSKY S92 A – ATA 2613 OVERHEAT DETECTION SYSTEM – HARNESS – Defective. DR 611953351
During initial takeoff acceleration, #2 fire warning light, tone sounded briefly and abruptly extinguished twice. Takeoff aborted, Emergency Checklist followed, return to base conducted. Root cause: Flame Detector-Harness Assembly defective. #2 outboard fire detector replaced IAW AMM 26-11-XX.
EUROCOPTER AS365N3+ – ATA 2910 HYDRAULIC SYSTEM, MAIN – HOSE – LEAKING. DR 611953435
Excessive hydraulic fluid found on the transmission deck. Hydraulic (PTFE Teflon) hose found to be leaking above its attachment fitting. Hose replaced with new item.
AGUSTA AW139 – ATA 6320 MAIN ROTOR GEARBOX – GEARBOX – CHIP INDICATION. DR 611953443
IGB chip caution illuminated, extinguished. 2 minutes later, IGB chip caution illuminated again. Trouble shooting carried out IAW 39-A-65-42-00-00A-421C-A, Fault Code 6542-03 noted. 5 small metal particles of much less than 1 mm IAW with 39-A-60-80-00-03A-028A-D detected. 39-A-65-42-00-00A-31AA-A, 39-A-65-42-00-00A-320A-A, 39-A-60-80-00-03A-028A-D referred. Aircraft can continue to fly IAW 39-A-60-80-00-03A-91HA-D. Ground run carried out nil particles found.
EUROCOPTER AS365N3+ – ATA 2590 NON STANDARD EQUIPMENT SYSTEM – CLUTCH – SLIPPED. DR 611953461
Extreme winch pendulum – wire caught RH Main Gear Tyre, wire ran down entry protective bar, possible clutch slip. Investigation into the circumstances is ongoing.
SIKORSKY S92 A – ATA 2310 HF COMMUNICATION SYSTEM – HF SYSTEM – FAILED. DR 611953465
While outbound, Pilot Monitoring conducted HF radio check. HF tuned continuously without channel lock. Multiple attempts were made to power off and retune, no help. Aircraft returned to base. All connectors and links to PA coupler cleaned, inspected and re-secured. Radio ground check carried out with Flight Watch, satisfactory. Assessed as serviceable IAW EO S92-23-XX.
SIKORSKY S76 A – ATA 2910 HYDRAULIC SYSTEM, MAIN – SERVO CONTROL – FAULT MESSAGE. DR 611953466
In flight, no #1 servo light illuminated. ECL actions were carried out, light was cleared. However, after 3 mins of flight time, light reappeared. ECL’s again consulted, aircraft diverted. Investigations are continuing.

Momentarily prior to commencing a winch circuit, #2 Generator failed. Referred to ECL. Generator would not reset after second occurrence. Returned to base. #2 Generator replaced.
