LYCOMING O320 D3G – ATA 8520 RECIPROCATING ENGINE POWER SECTION – CYLINDER – CRACKED. DR 611953401 (image above)
Rough running engine. Cylinder head cracked. Cylinder assembly renewed.

LYCOMING IO360 L2A – ATA 8520 RECIPROCATING ENGINE POWER SECTION – EXHAUST VALVE – BURNT. DR 611953415
Low compression No 1 cylinder, due burnt exhaust valve face.Valve replaced and seat re cut.

LYCOMING IO540 K1A5 – ATA 7322 FUEL CONTROL/RECIPROCATING ENGINES – FCU – Out of adjustment. DR 611953423
Uncommanded engine shut down at idle when mixture was moved to full rich and aux pump was turned on just prior to commencing take off roll.
The engine was replaced only 48.7 HRs prior and the fuel components settling in caused the idle mixture to increase and the idle RPM to decrease. When the mixture was moved to full rich and aux pump was simultaneously switched on it caused the engine to rich cut.
A ground run revealed the engine idle RPM to be low (approx. 525RPM) and the idle mixture to be rich (200RPM rise when leaned with aux pump on). The FCU was subsequently adjusted to correct idle mixture and RPM. Engine ground run was c/o to confirm correct operation and correct parameters across the entire operating range.

LYCOMING O360 E1A6D – ATA 8500 ENGINE RECIPROCATING – PISTON ENGINE – FAILED. DR 611953456
Sudden engine failure in-flight. Engineering investigating.
