LYCOMING IO540 K1A5 – ATA 8550 RECIPROCATING ENGINE INTERNAL OIL SYSTEM – OIL COOLER – CRACKED. DR 611953541

4.3 flight/hrs after engine installation, oil leak was discovered originating from RH oil cooler. Inspection of removed RH oil cooler determined it was a pre-existing condition. Cooler replaced. Throttle cable bracket was removed, inspected, adjusted and re-installed in the correct position.

During training flight, an unusual noise and engine vibration was noted. Continued circuit and landed. Root cause: Exhaust riser had separated on flange of the engine’s number two cylinder. Number two exhaust riser was replaced with new item. Aircraft returned to service.
