CESSNA 404 – ATA 3260 LANDING GEAR POSITION AND WARNING SYSTEM – SWITCH – FAULTY. DR 611953693
Gear unsafe light on after retract.

Root cause: Right-hand up lock switch unserviceable.

Corrective action: Right-hand up lock switch replaced with new P/N 602EN55-6B. Left-hand up lock switch adjusted in accordance with the Cessna 404 service manual.

Retractions carried out, serviceable, hydraulic reservoir fluid level OK.

PIPER PA28 161 – ATA 2432 BATTERY/CHARGER SYSTEM – BATTERY – DISCHARGED. DR 611953697
Loss of radios in flight.
New battery fitted with all alternator checks; operations normal.
GIPPSLAND GA8 – ATA 5210 PASSENGER/CREW DOORS – ACTUATING ROD – bent. DR 611953698
Actuating rod found slightly bent when carrying out Service Bulletin GA8-2005-23 Issue 4 Part C.

No handle with an integral stop to prevent over rotation installed.

SB-GA8-2005-23 Issue 4 was released to address the issue with over-rotating handles. The rod inspection was previously carried out in January-2019 and the rod was replaced because a bent rod was found.

Loss of control in flight. Control regained and aircraft landed safely.
Structural failure on right-hand stabiliser Part # 0432001-59. Suspected Part # 0432001-54 Skin /Spar failure. Tail plane to be dismantled to look for existing damage.
SWEARINGEN SA227 DC – ATA 3010 AIRFOIL ANTI-ICE/DE-ICE SYSTEM – WING – OVERHEATED. DR 611953726
Right-hand wing overheat illuminated in flight. Bypass P3 pipe in Right-hand leading edge convolutions cracked. Pipe assembly replaced.
DIAMOND DA40 NG – ATA 2820 AIRCRAFT FUEL DISTRIBUTION SYSTEM – FITTING – FAILED. DR 611953749
Aircraft had a large fuel leak from left stub wing access panel.

Root cause: End fitting of the flexible hose Diamond P/N A23865-06 found to be separated from hose but fitting retained the outer green braided sleeving of hose assembly. Diamond aircraft has been notified of the defect.

CESSNA A152 – ATA 2720 RUDDER CONTROL SYSTEM – TERMINAL – CRACKED. DR 611953754
Both left-hand and right-hand rudder cables suspected to have cracked AFT terminals.
Both left hand and right-hand rudder cables replaced with new in accordance with Cessna 152 maintenance manual and McFarlane compatibility chart.
CESSNA 441 – ATA 5312 FUSELAGE MAIN, BULKHEAD – DRAIN HOLE – BLOCKED. DR 611953774
Pilot reported that elevator and rudder trim was difficult to operate at low outside air temperature.
Root cause: Tail/Empennage drain holes blocked.
At high altitude, frozen water is suspected to cause reported defect.
Drain holes cleared.
