|During taxi, fuel pressure dropped off and engine started to run rough. Defect rectified by replacing engine driven fuel pump. Teardown report has been requested from vendor.|
|LYCOMING O320 D3G – ATA 7322 FUEL CONTROL/RECIPROCATING ENGINES – CARBURETTOR – LEAKING. DR 611954142|
|Carburettor found to be leaking during routine leak check. Further investigation found a hole in the casting between the bottom of the fuel bowl and the lower throttle body area.
Carburettor replaced with serviceable item.
|LYCOMING IO540 M1A5 – ATA 8520 RECIPROCATING ENGINE POWER SECTION – CYLINDER – LOW COMPRESSION. DR 611954157|
|During inspection, cylinder compression #1 check failed. Cylinder removed and a crack was found in inlet port extending from inlet seat. There were signs of pre-ignition / detonation as well.
Could be possible issue with fuel and high CHT ops. Have amended company operations to reduce CHT.

|#1-cylinder compression checked – cylinder failed. Cylinder cracked at inlet port originating at valve seat.|
|LYCOMING O320 E2D – ATA 7421 SPARK PLUG/IGNITER – SPARK PLUG – FAULTY. DR 611954195|
|Engine rough running.
Centre electrode on no. 4 lower spark plug missing, combined with fouling of no. 4 top plug, it resulted in cylinder not firing correctly.
Resolution: No.4 top spark plug cleared of debris; lower spark plug replaced.
Engine ground runs carried out, nil defects evident, engine ops normal.
|CONTINENTAL GTSIO520 M – ATA 8550 RECIPROCATING ENGINE INTERNAL OIL SYSTEM – OIL LINE – CRACKED AND LEAKING. DR 611954201|
|While conducting aerial survey, excessive oil was observed on the right hand engine cowling. Engine was shutdown as a precaution.
Root cause: Cracked inlet oil pipe.
