TURBINE ENGINE
GENERAL ELECTRIC CT7 9B –  ATA 7230 TURBINE ENGINE COMPRESSOR SECTION – COMPRESSOR – STALLED. DR 611953819
Inflight, as power was reduced, LH engine compressor stalled and the ITT over temperature light came on. Stall remained until corrective action was taken.

Relevant maintenance completed. Engine placed on 200 HR repeat inspection interval. The second stall event investigation is on-going.

PRATT AND WHITNEY PT6A 42 –  ATA 7420 IGNITION HARNESS (DISTRIBUTION) – IGNITION LEAD – BURNT. DR 611953822
L/H ignition lead end u/s. Burn out over time. Lead replaced.
PRATT AND WHITNEY PT6A 42 –  ATA 7200 ENGINE (TURBINE/TURBOPROP) – STATOR – CRACKED. DR 611953823
1 – Oil pressure transducer leaking.
2 – 1 x T5 Probe u/s. No response to heat during test at HSI
3 – Power Turbine stator found cracked, found at HSI – Allowable
4 – Combustion Liner cracked, found at HSI
5 – Ignition unit trips CBs when switched on
6 – Power Turbine stator cracked – Allowable – Reinspection required.
7 – ITT Harness chaffed
Rectifications completed.
LYCOMING ALF502R 5 –  ATA 7321 FUEL CONTROL/TURBINE ENGINES – ACTUATOR – FAILED. DR 611953824
During taxi-in, TMS indicated a ‘white chevron down’ on #1 Engine after a normal flight. Engine still running after shutdown. Crew confirmed that MAX TGT recorded throughout did not exceed 700*C.

Root cause: TMS actuator found to be extended, causing FCU to continue to put fuel into the engine. TMS actuator replaced.

Tail pipe & associated areas inspected for heat damage IAW AMM 78-10-00 & 70-00-00 – nil defects evident.
MAX TGT did not exceed 882*C {Per AMM 72-00-00 fig. 634}. Follow up maintenance complete.

4 – Power turbine stator cracked – Allowable – found @ HSI
5 – Metal in oil found @ HSI
6 – Metal in oil – 10 hour inspect
7 – Metal in oil – 50 hour inspect
8 – Metal in oil
GENERAL ELECTRIC GENX1B64 –  ATA 7250 TURBINE ENGINE TURBINE SECTION – Blade Slot – PARTIALLY SEPARATED. DR 611953869
During disassembly of engine at shop level, an abnormal finding on HPTS2 disk was detected.  As a result of this, HPT Module has been quarantined subject to OEM investigation.
 
GARRETT TPE331 12UHR –  ATA 7260 TURBINE ENGINE ACCESSORY DRIVE – GEARBOX – FAILED. DR 611953874
Uncommanded engine shutdown in flight.
Root cause: Accessory gearbox failure. Fuel pump and FCU removed, no drive output from accessory gearbox.
Engine replaced.
ROLLS ROYCE 772B-60 –  ATA 7321 FUEL CONTROL/TURBINE ENGINES  – Seal Plate – Fuel leak. DR 611953894
Inflight, fuel leak was identified on #2 engine, #2 engine was shut down as a precaution, fuel leak ceased. ATB performed.

Root cause: Seal from seal plate on inlet to fuel flow transmitter protruding. Moreover, upper two inlet flange bolts found loose. Seal plate replaced IAW AMM 73-31-11-400-803-A.

Investigation initiated.

HONEYWELL LTS101 700D2 –  ATA 7250 TURBINE ENGINE TURBINE SECTION – BOLT – SHEARED. DR 611953903
During visual inspection, one of the turbine blade retainer bolts head was found to be sheared off.
OEM advised.

Odd fumes smelt on take-off, then for 30 minutes after departure and again on descent.

Root cause: Suspected smell from compressor wash fluid.

Compressor water washed / rinsed. Engine ground run carried out with power levers moved through ranges with no smell evident.
