AIRBUS A320 232 – ATA 3241 BRAKE ANTISKID SECTION  – Anti Skid System – FAULT MESSAGE. DR 611954567
Low speed take off abort at approximately 30 kt due Brake Sys 2 ECAM. Fault reset via anti-skid switch. TSM procedure of BSCU reset actioned. A/C serviceable.
SAAB 340 B – ATA 3230 LANDING GEAR RETRACT/EXTENSION SYSTEM – SWITCH – FAULTY. DR 611954568
After take-off, Gear selected up, main gear Green lights went out, Nose gear Green light stayed illuminated. Gear cycled with same indications.

Root cause: Nose down lock switch faulty. Switch removed, switch’s plunger noted to be slightly bend, rusted and when exercised, found sticky.

Nose landing gear down lock switch/hardness replaced. Extension and retraction test carry out several times. Nil fault evidence.

ATR ATR72 212A – ATA 3230 LANDING GEAR RETRACT/EXTENSION SYSTEM  – RELAY – FAULTY. DR 612054579
After take-off gear was selected up, 3 gear unsafe lights as well as 3 Green gear arrows illuminated on both primary and secondary indications. QRH checklist actioned, ATB to base executed.

Root cause: Failure of ‘15GA’ pos up CTR relay. ‘15GA’ pos up CTR relay found not to be providing earth to the pin F of L/G selector valve. ‘15ga’ relay was replaced and tested ok.

No current reliability concerns.

ATR ATR72 212A – ATA 3230 LANDING GEAR RETRACT/EXTENSION SYSTEM  – LANDING GEAR – FAILED-RETRACT. DR 612054585
In flight, landing gear was unable to retract on take-off, 3 Green arrows and 3 gear unsafe lights illuminated when the gear selected up. QRH actioned, ATB to base planned.
Repeated defect, currently AOG awaiting detailed troubleshooting.
 
BOEING 737 8FE – ATA 2530 GALLEY STATION EQUIPMENT SYSTEM  – OVEN – Burning Smell. DR 612054587
Cabin crew reported suspected electrical burning smell from FWD Galley oven. Oven switched off and CB pulled out, smell went away.

Root cause: Faulty steam oven, same replaced.

Subject PN is a condition monitoring component, and its function is checked every time when in use. The event is considered random, no further action is required at this time.

BOEING 757 225 – ATA 5610 FLIGHT COMPARTMENT WINDOWS – WINDSHIELD – ARCED. DR 612054570
RH windshield arcing observed on small section on bottom of shield. No other indications. Suspect part Internal failure.

Relevant wiring checks carried out; window heat control unit passed all tests but being replaced as a precaution. Nil arcing evident on J1, J4 & J5 Connectors. However due previous history, J1 and J5 have been replaced. J4 will also be replaced as a precaution (ref AD 2012-25-03). Vendor review/repair strip report of windshield will be requested.

 
AIRBUS A320 232 – ATA 2100 AIR CONDITIONING SYSTEM – BLEED AIR – CONTAMINATED. DR 612054595
Crew reported cabin odour at end of flight whilst on bay.
Engineers were unable to reproduce or confirm, however as a precaution, APU placed on MEL, bleed system operated from main engines with nil findings, aircraft released to service.
BOEING 777 3ZGER – ATA 3445 TRAFFIC COLLISION AVOIDANCE SYSTEM (TCAS) – COMPUTER – FAILED. DR 612054597
In flight, crew reported TCAS failed during flight.
Root cause: Failure of TCAS computer. Same replaced.
PN in question has no reliability concerns.
 
BOEING 717 200 – ATA 5755 SPOILERS  – SPOILER – DISTORTED. DR 612054609
Damage found on RH wing upper surface at the hinge line of the inboard spoiler. Spoiler found to be distorted, as had tear approx. 10cm long and 2 pulled rivets.

Boeing approved repair embodied. Moreover, spoiler assy replaced.

FOKKER F28 0070 – ATA 2510 CREW STATION EQUIPMENT SYSTEM  – SEAT – CRACKED. DR 612054622
Multiple cracked spine assemblies reported on crew seats in workshop. Suspected to be caused by fatigue and age.

On investigation, through disassembly of the crew Seat and subsequent un-riveting of the Spine Assy, it appears that along the way, there have been attempts made to repair certain accessible cracks with a doubler.

The manufacturer, Ipeco, through the CMM 25-11-04, state that no repairs are permitted and to contact Ipeco for a repair. Rectifications are continuing.
