TURBINE ENGINE
GARRETT TFE731 2 –  ATA 7200 ENGINE (TURBINE/TURBOPROP) – TURBINE ENGINE – FLUCTUATES. DR 611954362
After take off pilot reported that the RH engine commenced fluctuating at approximately 500 ft. Checklist procedures carried and engine brought to idle.  Pan declared; fuel dump carried out. Aircraft returned safely to base without further incident.
Engineering investigating.

After take-off, torque was at 99%. At 1500ft RH torque rolled back by 3% on the gauge but could not be felt by the pilot on the aircraft. During execution of RH turn, at about 4000ft, RH bypass light came on.

Initial reports indicated EGT increasing well and beyond 650 C. Engine shut down without pilot’s interaction. After landing, on ground pilot reported tail pipe of RH engine was significantly more wet with what appeared to be fuel than the LH engine.

Engineers reported there was considerable associated damage because of vibrations to the engine and propeller. Engine is currently being replaced.

PRATT AND WHITNEY PW123 E –  ATA 7250 TURBINE ENGINE TURBINE SECTION – TURBINE ENGINE – FAILED. DR 611954404
On application of Take off Power for static departure, #2 engine Failed.
#2 torque gauge failed, NP dropped to Zero. Core engine readings remained near normal.
Loud bang was heard on board, tower reported smoke observed from Right tailpipe
#2 engine torque codes 18,13,14,32,33 recorded flags noted on engine condition panel ,Oil Tank MCD & #2 engine ECU.
Visual inspection carried out down tailpipe to inspect Power turbine, extensive PT section damage noted , metal fragments/Debris  recovered from  runway ,root cause not determined and engine requires replacement.
Investigation ongoing.
GENERAL ELECTRIC CT7 9B –  ATA 7200 ENGINE (TURBINE/TURBOPROP) – TURBINE ENGINE – OVERTEMPERATURE. DR 611954412
Rejected take off carried out due to LH ignition light on and yaw to the left, followed by LH engine over temp light on. LH engine ITT increased rapidly to above 1100 degrees ITT, until LH condition lever set to fuel off. Passengers observed flames exit the LH tailpipe. Single engine taxi to the bay.
