BOEING 737 8FE – ATA 2590 NON STANDARD EQUIPMENT SYSTEM  – PRINTER – Burning Smell. DR 611954229
Whilst printing an ACARS message, burning smell was noticed to be coming from the printer area. Printed paper had a burn mark down on left-hand side as well. The printer turned off.

Root cause: Failure of flight deck printer PN 700750-112.

Flight deck printer replaced. Considered a one off defect.

BAE AVRO146RJ 85 – ATA 2697 FIRE PROTECTION WIRING – Discharge head – Wiring Defective. DR 611954246
On landing, ENG 2 EXT #2 USED warning caption illuminated – as did ENG 1 FIRE HANDLE caption.

Operational check of #1 LP (fuel shut off) + fire handle checked satisfactory.

Root cause: Intermittent ‘Short circuit’ found in #2 ENG, #2 extinguisher cylinder, discharge head wiring.

Cylinder/head assy replaced, tested OK IAW AMM 26-20-XX.

ATR ATR72 212A – ATA 2510 CREW STATION EQUIPMENT SYSTEM  – SPRING – BROKEN. DR 611954250
When carrying out the scheduled maintenance task “XX” for the compliance of EASA AD 2016-0256, one of two track lock springs on the captain seat was found broken.

Both springs replaced, refer IPECO SB 063-25-14.

MDR raised for reporting purpose of AD findings. No further action is required.

BOEING 717 200 – ATA 4990 APU OIL SYSTEM – APU – OVERFILLED. DR 611954253
Oil smell evident in cabin, on ground with just APU air ‘on’. Smell dissipated when APU air turned ‘off’, returned when APU air re selected.

Relevant maintenance completed, APU tank oil quantity, found to be above the “full” mark + oil appearance noted as ‘suspect’.

APU oil system drained & filled to correct service mark. Close up done.

DE HAVILLAND DHC8 202 – ATA 2913 HYDRAULIC PUMP, (ELECTRIC/ENGINE), MAIN  – PUMP – FAILED. DR 611954264
15 minutes prior to top of descent, ‘#2 HYD FLUID HOT’ caution light illuminated. Associated QRH checklist was completed, fuel was reviewed and decision to divert was made. Alternate gear extension carried out; aircraft landed safely.

Root cause: Internal failure of Engine Driven hydraulic pump. Case drain filter also found by passed. EDP replaced.

Hydraulic pump sent for strip examination.

BOEING 737 8FE – ATA 5753 TRAILING EDGE FLAPS  – SKIN – DISBONDED. DR 611954283
During daily check, LHS outboard trailing edge flap lower skin was found disbonded at inboard end. LHS outboard TE aft flap was replaced IAW AMM 27-51-22-400-XX.
Investigation initiated.
DE HAVILLAND DHC8 402 – ATA 3246 WHEEL/SKI/FLOAT  – Tread – FAILED. DR 611954285
Left inboard tyre tread blew off during landing roll causing secondary damage to the under carriage.

 

Two similar defects reported

FOKKER F28 0070 – ATA 3240 LANDING GEAR BRAKE SYSTEM  – SLEEVE – LEAKING. DR 611954287
During closing of modification task on park brake shut off valve, swagged sleeve failed pressure test. Identified as a HF issue, correct sleeve installed.
IAI 1124 – ATA 2913 HYDRAULIC PUMP, (ELECTRIC/ENGINE), MAIN  – SEAL – DAMAGED. DR 611954290
On descent, pilot noticed fluctuating oil pressure + intermittent low oil pressure warning. Power reduced and uneventful landing made.
Hydraulic pump seal PN 3020052-2 on L/H Engine found to have failed. This was due for replacement at MPI in 83 hours. As per Honeywell SB TFE-72-3759R01 Honeywell are fully aware of the incident. New seal PN 3020052-26 is being installed as per SB.
BOEING 717 200 – ATA 5730 WING, PLATES/SKIN – SKIN – CORRODED. DR 611954304
Corrosion evident on RH Wing upper skin, at numerous areas. Affected areas previously repaired on a Temporary basis. ICA follow up action led to the discovery of additional corrosion finding. Reporting of corrosion extent to Boeing underway, awaiting OEM response.
