Description During the final approach, the crew of the Tecnam Sierra observed the Cessna 206 in close proximity on final approach. The crew of the Sierra conducted a missed approach to regain separation.

Description The paraglider collided with terrain and the pilot sustained serious injuries.

Description During search and rescue operations, the helicopter crew commenced a manual deceleration from 800 ft to locate an emergency beacon. The auto hover switch was engaged at about 600 ft. The crew subsequently detected a nose high attitude and an increasing rate of descent with reducing airspeed. The pilot applied power and forward cyclic, arresting the rate of descent at about 90ft.

Description During climb, the engine ran rough and subsequently lost power resulting in the crew conducting a forced landing. The pilot and two passengers sustained minor injuries.

Description During cruise, the engine ran rough and lost partial power and the crew conducted a forced landing on a beach. The engineering inspection revealed a crack in the crankcase at the base of cylinder number six.

Description During take-off, the aircraft collided with terrain and was destroyed by fire. The two crew sustained serious injuries.

Description The aircraft bounced and landed hard. The landing gear collapsed resulting in substantial damage to the aircraft.

Description During landing, the pilot lost directional control and the gyrocopter rolled over resulting in substantial damage. It was determined foreign object debris was the cause of the loss of control.

Description During landing, the aircraft bounced multiple times and landed hard. The nose landing gear partially collapsed resulting in substantial damage.

Description During final approach, the pilot inadvertently retracted the landing gear resulting in a wheels-up landing. The aircraft sustained minor damage.

Description During a touch-and-go landing, the pilot retracted the flaps soon after take-off, resulting in the aircraft sinking onto the runway. The pilot then rejected the take-off, veered off the runway and collided with a sign, resulting in minor damage. The pilot subsequently crossed runway 35L on foot, resulting in a runway incursion.

Description During aerial agricultural operations, the aircraft struck a powerline.

Description During cruise, the engine failed and the pilot conducted a forced landing. The engineering inspection did not reveal any faults with the engine.

Description During descent, the number two engine lost power. The crew shut down the engine and reduced power to number one engine to counter the yaw. The crew conducted a forced landing in a paddock resulting in substantial damage to the aircraft.

Description After landing with the engine running, the pilot exited the helicopter. The helicopter began to power up and subsequently took off with no one on board. The tail rotor struck the ground and the helicopter collided with terrain resulting in substantial damage.

Description During circuit operations, the Cessna 152 turned ahead of the proceeding Aeroprakt A22LS Foxbat in close proximity. The controller instructed the Cessna to climb to increase separation.

Description During final approach, the aircraft crashed into terrain. The aircraft was destroyed and the pilot, who was the sole occupant, was killed.

Description During landing, the aircraft overran the runway and collided with a fence resulting in minor damage.

4 COMMENTS

  1. Initial notifications but little knowledge to be gained from the report to the benefit of other pilots.

  2. Two very similar incidents in GA-8s, on the same day, at opposite ends of the country….?!

  3. Not too sure what the point was of all that? No pilot is going to benefit from others mistakes from virtually no details!

  4. I benefit …don’t let your guard down,Complacency easily sets in These accident reports are a reality check.Similar to road side memorials.
