Description During initial climb, the aircraft hit a powerline and crashed into water resulting in fatal injuries to the sole occupant.

Description During climb, the engine ran rough and the crew noticed fumes in the cockpit. The engine subsequently failed, and the crew made a forced landing in a paddock. Post-flight, the crew observed three-cylinder head temperature indications to be in the red.

Description. During landing, the landing gear spreader bar failed resulting in the wing tip hitting the ground. The aircraft came to rest upside down and was substantially damaged.

Description During landing, the tail rotor hit the ground and separated from the helicopter resulting in a loss of control and heavy landing. The helicopter sustained substantial damage.

Description During initial climb, the engine lost power and the pilot made a forced landing on a nearby golf course resulting in minor damage. The two-crew sustained minor injuries.

Description During cruise, the engine lost partial power and the pilot made a forced landing in a paddock. During the landing, the engine lost all power. The engineering inspection revealed the exhaust valve failed resulting in damage to the cylinder and piston.

Description During take-off, it is reported that the aircraft failed to accelerate, and was rejected. When brakes were applied, the right main landing gear collapsed and the pilot lost directional control, overrunning the runway. The aircraft sustained substantial damage.

Description The pilot hand swung the propellor to start the engine, with no-one at the aircraft controls. Following the hand start, the aircraft began moving. The aircraft subsequently collided with a retaining wall resulting in substantial damage to the aircraft and minor injuries to the pilot.
