
SDR 510018368 Jabiru 1600A Reciprocating engine cylinder section valve seat separated. Engine exhaust valve seat dropped. Engine had numerous other problems leading up to seat separation. Many other problems found when engine disassembled for repair.

SDR 510018751 Boeing 717-200 Aerodynamic fairing lever incorrect fit. Tail cone release lanyard for aft emergency slide incorrectly routed. Further investigation found the release cam lever (pin) was fitted 180 degrees out and the lanyard was attached to the guide/bracket instead of to the lever. Fleet investigation found another two aircraft with the same incorrect installation. P/No: 3961567501

SDR 510018650 Continental IO240B Engine starter motor failed. Starter motor failed during start. Starter drive gear sheared. Inspection of crankshaft gear found damaged beyond limits. P/No: BC3201. TSN: 103 hours

LH main landing gear lower yoke fitting sheared causing LH landing gear wheels to turn sideways and skid along runway, causing aircraft to veer to the left and leave the runway and enter the sand.
