AIRBUS A320 232 – ATA 5320 FUSELAGE, MISCELLANEOUS STRUCTURE – CENTREPOST – CRACKED. DR 611953706
During base inspection, cockpit window centre post lower continuity fitting left-hand and right-hand were found cracked on visual inspection. Non-destructive testing was carried out. Crack was confirmed and reported to Airbus for repair instructions. Airbus has mandated a replacement.
AIRBUS A320 232 – ATA 3222 NOSE/TAIL LANDING GEAR STRUT/AXLE – CYLINDER – CORRODED. DR 611953709
During nose landing gear dynamic seals replacement, corrosion was found on lower edge of inner cylinder, upper and lower cam. Corroded parts replaced.
BEECH 1900 B200 – ATA 2710 AILERON CONTROL SYSTEM – CABLE – DAMAGED. DR 611953711
During heavy maintenance, an inspection of the cable runs on the control column showed that the chain and cable assembly part No 100-524123-5 were incorrectly routed over a cable keeper. Moreover, aileron control column cable was found to be rubbing on a cable-retaining pin as well. Inspection revealed that there was worn and frayed strands on the cable where it was rubbing on the cable keeper.

Chain and Cable Assy have been replaced with a new item.

BOEING 787 8 – ATA 5720 WING, MISCELLANEOUS STRUCTURE – D rib Aux – CRACKED. DR 611953724
During check C, left-hand flap support Aux D backup ribs were found cracked.
AIRBUS A320 233 – ATA 5420 NACELLE/PYLON, MISCELLANEOUS STRUCTURE – SPACER – PART DEPARTED Aircraft. DR 611953725
A spacer was located on the departing runway, latter established to be belonging to engine tail drain mast. Bracket underneath it was also found corroded.

Both, the pylon drain support assembly and the clamp block assembly from the effected engine were replaced in accordance with the relevant aircraft maintenance manual procedures.

AIRBUS A320 232 – ATA 7160 ENGINE AIR INTAKE SYSTEM – ACOUSTIC MESH – DELAMINATED. DR 611953736
During schedule maintenance, substantial section of the number 1 engine acoustic woven wire surface detached from the parent honeycomb material (Inlet Cowl).

Evidence of the lining debris has entered the core (LPC) with remnants of the woven wire wrapped around the inlet guide vanes.

Engine LPC inspection performed, inlet cowl replaced.

BOEING 717 200 – ATA 3242 BRAKE – Return Spring – BROKEN. DR 611953737
During scheduled maintenance, brake unit return spring/housing found damaged.

Adjuster pin retainer hole elongation by wear. The adjuster pin is not properly engaged anymore and can disengage. This will let the adjuster spring housing migrate and interfere with the landing gear.

Preventive measures: Check for retainer slot wear. An adjuster pin can serve as go/no-go by testing its engagement at each retainer location. These recommendations were forwarded to overhaul facility for incorporation during workshop visit.

Brake unit replaced, inspection of surrounding structure and MLG completed.

AIRBUS A330 202 – ATA 4950 APU BLEED AIR SYSTEM   – BLEED AIR – CONTAMINATED. DR 611953753
On arrival, with APU bleed selected on, an oil type smell was noted in the cockpit. Engineering troubleshooting confirmed oil smell only present with APU bleed air selected on. Subsequently, MEL 36-12-01A applied to APU bleed air supply system.

Engineering investigation of the contaminated air supply in progress.

FOKKER F28 0100 – ATA 2731 ELEVATOR TAB CONTROL SYSTEM   – Elevator servo capstan – Jammed due Icing. DR 611953766
During descent, while passing approximately 15,660 feet, with autopilot 1 engaged, crew encountered an uncommanded nose down movement.

Root cause: Insufficient grease application to system 1 identified. System 2 considered serviceable. Possible icing event.

Lubrication of elevator servo-motor system was carried out, systems were declared serviceable.

Resolution: Improper lubrication of servo-motor system due to human factors. Maintenance teams training refreshers arranged.

BOEING 737 7FE – ATA 2760 DRAG CONTROL SYSTEM   – CABLE – DISCONNECTED. DR 611953773
On rotation, speed brake did not arm. Flaps up config warning (light & horn). ‘PSEU’ light illuminated on touchdown as well.

Root cause: Ground spoiler interlock valve and cable disconnected at lower slider assembly on RH MLG.
Lower slider assembly was refitted, follow up checks satisfactory.

Investigation in progress
