ROBINSON R44 – ATA 6320 MAIN ROTOR GEARBOX – GEARBOX – CONTAMINATION-METAL. DR 611953733
Pilot felt vibration and loss of rotor dual tacho, landed immediately. On inspection, Hall effect senders found damaged with excessive pinion shaft movement. Inspection found large amounts of ferrous material on chip detector as well.
GUIMBAL CABRI G2 – ATA 7721 CYLINDER HEAD TEMP (CHT) INDICATING SYSTEM – PROBE – BURNT. DR 611953694
Exhaust system removed for engine removal and probe found unserviceable. Probe section burnt way.
GUIMBAL CABRI G2 – ATA 5302 ROTORCRAFT TAIL BOOM – FITTING – CORRODED. DR 611953710
Tail boom removed for 500-hour inspection. Lower right-hand fittings found corroded. Fittings replaced.

At 100-hour inspection, there was metal on the MRGB chip pug. The light had not been activated and the chip plug was tested satisfactory. A borescope inspection was carried out per HG MM 21-1-12. There is evidence of corrosion pitting on at least 3 of the teeth.
The manufacturer was informed and asked for the gearbox to be replaced. An overhauled MRGB was installed.One similar report received

At 50-hour inspection, cracked found in the corners above the canopy slat outer attachment points.
