Missing section on hoist level wind screw. (611954032)
ROTORCRAFT
EUROCOPTER AS350 B3 – ATA 6220 MAIN ROTOR HEAD – SPRING – CRACKED. DR 611953980
Crack found in one spring of the vibration dampener assembly. Spring set replaced.
SIKORSKY S92 A – ATA 3297 LANDING GEAR WIRING  – WIRE – CHAFING. DR 611954002
Selected landing gear down, none of the three green indication lights illuminated, red unlocked light remained on. Landing Gear control lever recycled with the same result and no change in number three hydraulic system indications. Landing gear control circuit breaker was identified open, circuit breaker reset, but opened again. Landing gear was lowered using emergency extension system as per aircraft checklist.
Root cause: A chaffed wire that created a short in the system.
AGUSTA AW139 – ATA 5350 AERODYNAMIC FAIRING  – FAIRING – Part Departed Aircraft. DR 611954009
During post flight inspection, hoist fairing was found to be missing along with 3 attachment screws. Hoist had a cable retention carried out prior to last flight.
Incident reported to safety.
SCHWEIZER 269C 1 – ATA 2432 BATTERY/CHARGER SYSTEM  – MASTER SWITCH – UNSERVICEABLE. DR 611954022
During pre-flight, battery master switch found defective. New switch installed, operations tested sat.
AGUSTA AW139 – ATA 2590 NON STANDARD EQUIPMENT SYSTEM  – Windscrew – DAMAGED. DR 611954032
Hoist level wind screw found with missing section on level wind screw in area where drum sits when it is in the park position on the aircraft.

Goodrich are suggesting this is from vibrations incurred whilst hoist is fitted to the airframe and sitting in the parked position – still awaiting on strip report from Goodrich.

GUIMBAL CABRI G2 – ATA 2844 FUEL PRESSURE INDICATOR  – Pressure sensor – FAULTY. DR 611954047
During pre-flight inspection, pilot turned on the master switch and noticed on the EPM self-test screen “fuel pressure sensor FAIL” warning coming up. The continue button was pressed and the digital read out on the fuel pressure indicated XXX. The RFM confirms this as a sensor fail.

New sensor installed and tested sat. Relevant SB19-001A mod 18-060. MM55A
