SWEARINGEN SA227 DC – ATA 3232 LANDING GEAR DOOR ACTUATOR – ACTUATOR – BROKEN. DR 611954371
LH MLG Actuator found Gland broken away from Main Body.

Actuator Replaced

PIPER PA28 181 – ATA 1210 FUEL SERVICING – FUEL SYSTEM – CONTAM-WATER. DR 611954381
Aircraft observed being refuelled in heavy downpour of rain.
Aircraft grounded until fuel system could be checked for contamination.
4/11/2019 fuel system checked with water evident.
Fuel system completely drained and flush with isopropyl alcohol.
System checked with all contamination cleared.
BEECH 58 – ATA 5341 FUSELAGE, WING ATTACH FITTINGS – HINGE – CORRODED. DR 611954409
Hinge half connecting RH wing lower skin to lower wing spar cap found with hinge lugs corroded and displaced, pushing away the lower wing skin.
Also evidence of severe fretting of forward wing spar hinge.
Aircraft has 12646 flight hours and 45 years in service.
Aircraft retired from service
 
BEECH 1900 B200C – ATA 8011 ENGINE STARTER – STARTERGENERATOR – FAILED. DR 611954357
Right hand starter generator fails to start engine. Cannot exceed 2% N1 compressor speed.
Starter generator replaced in field.
Investigation underway – supplier to provide strip report of failed unit.
 
PILATUS BN2B 26 – ATA 7810 ENGINE COLLECTOR/TAILPIPE/NOZZLE – EXHAUST PIPE – LEAKING. DR 611954366
Throttle/Prop controls become stiff, seizing up approx 9 mins from landing. Pilot reduced power on that engine as much as possible and conducted a normal landing with shutting the affected engine down at the flare.
Initial investigations show an exhaust clamp had failed allowing the exhaust to slightly separate behind the carby-heat shroud which forced the hot exhaust gas onto the affected cables.
Further examination ongoing.
