Inlet valve failure. (611954266)

LYCOMING IO540 AE1A5 –  ATA 8520 RECIPROCATING ENGINE POWER SECTION – EXHAUST VALVE – LEAKING. DR 611954247
On carrying out 100 hourly inspection, noted #1-cylinder low compression 20/80.

Removed cylinder #1 and replaced with new 05K21745 cylinder kit. Then cylinder assemblies all above 70/80 compression.

Carried out a borescope inspection check of the remaining cylinder assemblies, all OK. #1 cylinder showing signs of valve to seat damage, wear.

New 05K21745 cylinder kit assembly fitted. This cylinder sent to workshop for repairs.

Engine failed to make rated power during pre- flight engine run ups.
No 6 Cylinder RH engine, inlet valve has broken / failed at the top of valve stem above retaining keys. Valve has fallen out and hitting the piston it was pushed back into the cylinder head.
Cylinder was replaced.

Cylinder leak down 15/80. Worn Exhaust Guide. Replaced with new item.
LYCOMING O320 E3D –  ATA 8520 RECIPROCATING ENGINE POWER SECTION – PISTON ENGINE – POWER LOSS. DR 611954292
Shortly after take-off, (crosswind leg) at approximately 700 ft AGL, severe vibration was experienced together with a drop in RPM to 1500 – 1700.  Aircraft capable of maintaining height and was manoeuvred close to the field with an uneventful landing made.

On the ground, aircraft described as running relatively smoothly until RPM increased above 1,000 where severe vibration was encountered.  Aircraft shutdown and quarantined pending engineering examination.

No 1 Cylinder failed compression check 25/80 PSI leaking at both inlet and exhaust. Exhaust valve face worn. Inlet valve build-up of carbon and eroded face. No1 cylinder was installed at Engine O/H. No 1 Cylinder replaced.

No2 Cylinder removed due failed compression check, 25/180 PSI. Exhaust valve wear noted as contributory.

Alternator not charging, engine running rough and smoking at low RPM.

Alternator brush block studs and connections were cleaned and retorqued.

Leaking inlet valve guide found to be the cause of the rough running/smoke. Cylinder replaced.
